Parent or Guardian Contacts

1. 1. I agree that I will pay the fees associated with registering upon submission of the application.
   2. I agree that I am required to pay in full for Summer Programs, and a 25% non-refundable deposit will apply if a refund is requested.
   3. A $45 NSF fee will be due if any form of payment is returned or unable to process.
   4. I agree that the email address(es) I provided at registration is/are to be used for communication reasons. It is the recipients’ responsibility to make sure all communications are read - RDA is not responsible for emails going to SPAM or PROMOTIONS. Please check these folders BEFORE inquiring about a missed communication. Add us to your contacts/safe senders lists.
   5. I agree to inform a studio director or office manager if my dancer(s) withdrawals from Rivertown Dance Academy by requesting and completing an RDA withdrawal form.
   6. I give consent that the name of my child, photos, and video footage taken during their participation at the Rivertown Dance Academy may be used for promotional advertising including use in newspaper articles and social media.

   If you do not agree to the above photo consent please indicate on the registration page under “photo consent.”

   1. Should I have any complaints or concerns regarding Rivertown Dance Academy I will contact a studio director (Catherine Cristofaro, Laura Dufour) or the office manager.
   2. The multi-class discounts apply to single students as well as siblings and parents. 
   3. Each receipt will be emailed to you and is considered legitimate for tax receipt purposes. Please keep them for your records.
   4. We accept cash or cheque, e-transfers (hello@rda.dance), debit or credit cards. Please make all cheques payable to Rivertown Dance Academy. 
   5. All applicable discounts will be taken off of the base class prices, excluding HST charges.
   6. All schedules filled out online by registrants are pending studio approval and will be confirmed by office staff via email.
   7. Any credits applied to your family dance account will expire during the registration season in the preceding dance season.
